Forms/Registration must be completed online: Smithfield Public Schools Athletics Registration

Required Athletic Forms/Information:

1. RIIL Middle School Assumption of Risk form
   It is only necessary to complete this form once during your child's middle school years. The form must be notarized. Our secretary is a notary public. 

2. Physical Health Form
   We MUST have a copy of your child's current physical on file. This form must be submitted annually based upon date of physical.

The following forms MUST be submitted each season:

1. Parental Permission and Acknowledgement of Athletics Policies Form

2. Authorization for Treatment/Proof of Insurance Form

3. Concussion Parent Signature Form
